SPORTS PHYSICALS—FMS STUDENTS
If your student plans on trying out or participating in an FMS or FHS sport next year, they must have a current physical. For your convenience, physicals will be given by local physicians on the following date:
January 24th—3:05—FMS Gym
Cost: $10.00 on MySchoolBucks under Sports Physicals (closes 1/21)
Parents/Guardians must fill out and sign the front of the form. The physicians fill out and sign the back of the form. Students report directly to the gym after school 1/24.
TURN IN FORM TO YOUR PE TEACHER BY JANUARY 21st
Portions of the physical will be done during PE classes on the 22nd and 23rd.
YOU MUST CREATE A DRAGONFLY ACCOUNT IMMEDIATELY
