Parents may use this convenient form when submitting an absence excuse.  You may submit the excuse to your child's teacher or the front office within three days of returning to school.  If you also have a doctor's excuse, attach it to this form.  If an excuse has not been submitted within three days of returning to school, the absence will default to unexcused.  

This is an optional form.  Excuses may be submitted by parent note or email to your child's teacher and/or registrar.
