DRAGONFLY 

All students who wish to participate in athletics must create a profile on DragonFly and complete the following forms:

• Participation Agreement (electronically)
• Concussion Information (electronically)
• Student/Parent Sudden Cardiac Arrest (electronically) 
• Preparticipation Physical Evaluation (electronically) 
• Physical Examination (Upload the back of the paper form) – This form must be completed, signed, and dated by a M.D. or D.O.
• Birth Certificate (upload the paper certificate)
• NFHS Sportsmanship Certificate (upload certificate after completion of the course)
