Parents and players need to have an account.  Follow the process on Dragonfly! It will alert you as to what is needed.

Per AHSAA rules, physicals must be done by a medical doctor.  Forms must be signed and dated by said doctor. 

They cannot be completed by a nurse practitioner or chiropractor.
