• All student athletes must meet academic and other eligibility requirements in order to participate in any sport.  All paperwork must be filled out electronically or scanned into the DragonFly Max app.  Both parent and student must create and link their accounts to each other.  Instructions on DragonFly are below.  The following documents must be completed and on file prior to a student-athlete participating in any practice.

    • Preparticipation Physical Evaluation Form / Digital or printed (Below)
    • Physical Evaluation / Must be signed, stamped and dated by a doctor (Below)

    • Concussion Information Form / Digitial
    • Participant Agreement, Consent and Release and Venue Form / Digital
